During your therapy visit the therapist needs to have as much access to your skin as possible to thoroughly evaluate and treat you. Please wear loose clothing for easy access. Woman can wear a sports bra or two piece bathing suit. You may choose to wear your undergarments if you feel comfortable. Your skin needs to be clean and dry so please don't use oils, creams or body lotions on the day of your visit for your treatment to be the most effective. Sheets and blankets are available for draping.
Initial assessment takes approximately 75 min, treatment sessions thereafter take between 45-75 minutes depending on the severity of your condition and the type of modalities performed. This slot is reserved especially for you. It is a good idea to arrive a few minutes earlier so that you have time to use the restroom and relax prior to your treatment.
A referral from your physician may be necessary if you intend to pursue reimbursement. You will need to bring a signed and dated doctors prescription indicating "Physical Therapy Evaluate and Treat" including the diagnosis for your condition. Please bring your photo ID and insurance cards for your first visit as well.
AOHPT is a Physical Therapy Practice and treatment sessions are billable to insurance. We are a Medicare Approved Provider and bill Medicare and your Supplemental Insurance directly. We are out of network with all Private Health Insurance Plans. If you are a Private Health Insurance policy holder please call your insurance for details on out of network benefits of your plan. As a courtesy we will verify your insurance benefits prior to your first visit per request. Private Payers-We offer 30% cash discount to private payers. We accept CASH or CHECKS. Please note that there is a $40 service fee for checks with insufficient founds. Payment/Coinsurance/copay amounts are collected at the time of service so please bring some form of payment with you. We are not set up for credit card transactions.
Yes we also have some paperwork for you. Please click here to download the forms, complete them at home and bring with you for your appointment. If you have difficulty downloading or do not have a printer please call us and we will mail them to you promptly. If you prefer to complete them in our office please arrive 15 minutes earlier.
IF you wish to change or cancel your appointment we require a minimum 24-hour advance notice. Anything less will result in a $50 fee charged to your account. It costs us money to make appointments available to you. Whether you attend or not we still accrue the expenses. We don't charge you the actual cost for that appointment but rather a mere $50 fee. We do not make money with this charge; it is only to act as a deterrent from making the last minute changes. Advance notice allows someone else (who needs it) time to reserve it in place of you. Please be courteous and responsible. Thank You.
